Detailed Overview

Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:

The Lead, Health Equity and Community Engagement supports the efforts to reduce health disparities and achieve health equity by engaging with equity-seeking communities and address health equity issues.


The position builds strategic partnerships with both internal and external partners to develop and promote innovative mechanisms for service design and delivery. The position supports the integration of health equity lens and Equity, Diversity and Inclusion (EDI) principles in the health system through enhanced community engagement and tailored interventions. Responsible for the planning, implementation, maintenance, and evaluation of community engagement and partnership initiatives in order to build system capacity. The Lead, Health Equity and Community Engagement will play a critical role in helping to identify disparities in health care access, service provision, satisfaction, and outcomes.


Responsibilities

  1. Initiates and manages projects that will support Fraser Health become a leader in health equity and community engagement by enhancing system capacity and building engagement skills, and tools.
  2. Leads the overall coordination of health equity related community initiatives across Fraser Health. Leads and fosters active community engagement in idea generation, planning, implementation, integration and evaluation.
  3. Develops a scalable, flexible and sustainable model for community engagement and partnerships. Develops mechanisms to ensure that community engagement is responsive to changes in diversity and to feedback from patients, and providers.
  4. Builds and oversees a community engagement model that enhances system capacity for targeted interventions, including supervision of volunteers as required.
  5. Identifies and develops collaborative partnerships with healthcare providers, professional organizations, not for profits, academic researchers, other levels of government, and community groups to promote community partnership initiatives, increase capacity for innovation, encourage community engagement in service design and delivery, and create awareness through education.
  6. In consultation with Corporate Communications develops and implements a multi-level communications strategy for diverse community engagement including internal communications and collaboration with other key partners on the development inclusive communication strategy.
  7. In consultation with partners, develops and/or contributes to a community engagement programs to better understand the patient, provider, setting and systems factors that influence implementation of best practice in community engagement and partnerships.
  8. Develops or oversees the development of initiative documentation, including action and decision logs to assess program implementation and the identification, management, and mitigation of risks using a structured risk analysis methodology. Develops detailed work plans outlining initiatives to be undertaken. Prepares reports, graphs, tables, briefing notes, presentations, issue papers and makes recommendations based on the data.
  9. Provides day-to-day support to the Equity, Diversity and Inclusion (EDI) department in all aspects of initiative management such as coordination of communication to all stakeholders; provides guidance to staff directly assigned to projects; identifies needed resources, facilitates resource procurement and monitors contractual relationships with external consultants.
  10. Explores and evaluates related innovations, best practices, and frameworks in other organizations, in consultation with internal and external FH partners and stakeholders. Contributes to the broader goals of the Population and Public Health program by providing support to other priority initiatives as required.
  11. Collaborates with internal and cross-regional colleagues to support understanding and advancement of strategies to address challenges and opportunities for equity-seeking communities.
  12. Attend and participate in community meetings as necessary, which may include the Local Immigration partnerships, Refugee readiness planning tables, neighborhood associations, etc. in the evenings and on weekends as needed.
  13. Supports the implementation and evaluation of Health Equity initiatives for the region by working collaboratively with community health specialist (CHS) teams.
  14. Performs other duties as assigned.

Qualifications

Education and Experience

Master's Degree in a health related discipline, including but not limited to Public Health or Health Administration. Three (3) to five (5) years of recent related experience in public health, program evaluation, program management and work with equity seeking populations including experience with public and/or population change initiatives, or an equivalent combination of education, training, and experience.

  • Demonstrated knowledge of change and project management principles coupled with a strong understanding of the public health and the healthcare system.
  • Demonstrated project planning and strategic planning skills.
  • Broad knowledge of public health policies, procedures, regulations, and legislative environment.
  • Knowledge of other health care disciplines and their role in health care.
  • Ability to operate a computer and effectively utilize a variety of software applications.
  • Ability to work effectively under pressure and with changing priorities and deadlines.
  • Physical ability to perform the duties of the position.
